Hope on the Horizon!

Horizon FoundationCatholic Charities, Diocese of Trenton is grateful to the Horizon Foundation for helping us to help others! For a third year, the Horizon Foundation for New Jersey has generously awarded a grant of $20,000 to Ocean Community Services for its Project Hope Health and Wellness Education and Prevention Services initiative.

Project Hope is designed to help economically disadvantaged residents of Ocean County effect changes in their health practices, live healthy lifestyles, identify potential health risks and obtain treatment, and develop a plan with a case manager to meet basic and health care needs.

Project HopeActivities include:

  • Free Health Screenings that provide early intervention healthcare and linkages with local healthcare providers when a medical condition is identified.
  • Healthy Living workshops to help participants’ lead healthier lives.
  • ESL Health Literacy workshop that helps limited English proficient participants acquire the basic medical information and vocabulary they need to maintain their health and the health of family members.
  • Case Management to help participants develop a healthcare plan, connect with benefit programs and health care providers in the community, meet their basic needs, including food security and housing stability, and address issues that threaten their well-being.

Over the last three years, 600 individuals have received one or more services through Project Hope’s Health and Wellness Education and Prevention Services component.
